Η θεραπεία με πλάσμα πλούσιο σε αιμοπετάλια (PRP) ανήκει στις θεραπείες ενός νέου κλάδου της ιατρικής της αναγεννητικής ιατρικής "regenerative medicine", στόχος της οποίας είναι να διεγείρει τους φυσικούς μηχανισμούς θεραπείας του ίδιου του οργανισμού για να αποκαταστήσει αθεράπευτες βλάβες.
Οι επιστήμονες
προσπαθούσαν για πολλές δεκαετίες να ανακαλύψουν την ουσία η οποία ενέσιμα θα
εγχύοταν στο σημείο της ιστικής και οστικής βλάβης για να την αποκαταστήσει,
επιταχύνοντας την ανάρρωση ή αποτελώντας την οριστική θεραπεία, αποφεύγοντας
έτσι την πρόσκαιρη αντιφλεγμονώδη αγωγή που δεν έλυνε το πρόβλημα η την
χειρουργική επέμβαση. Το PRP πιθανόν να αποδειχθεί οτι είναι η λύση αυτή.
Ξέρουμε οτι ο
οργανισμός χρησιμοποιεί ένα συστατικό του ίδιου του αίματος σχεδιασμένο να
ενεργοποιεί τον μηχανισμό επούλωσης των βλαβών όταν αυτές προκύπτουν. Το
πρόβλημα όμως παρουσιάζεται, όταν πολλές φορές η περιοχή της βλάβης δεν μπορεί
να προμηθευτεί αρκετό απο το υλικό αυτό, τους αυξητικούς παράγοντες1,
και η διαδικασία είτε προχωράει αργά είτε σταματάει. Αυτό αποτελεί μία από τις
βασικές αιτίες που ορισμένες ιστικές βλάβες μεταπίπτουν σε χρονιότητα με συνεχή
πόνο και ενόχληση.
Οι αυξητικοί
παράγοντες βρίσκονται αποθηκευμένοι σε κύτταρα του αίματος που ονομάζονται
αιμοπετάλια, γνωστά για την συμμετοχή τους στην διαδικασία πήξης του αίματος.
Όταν τα κύτταρα αυτά ενεργοποιηθούν, απελευθερώνουν μαζικά τους αυξητικούς
αυτούς παράγοντες. Το σκεπτικό της θεραπείας με το PRP βασίζεται στην εξής απλή
συλλογιστική: εάν παίρναμε μια μικρή ποσότητα αίματος από τον ίδιο τον πάσχοντα
και από το αίμα αυτό δημιουργούσαμε ενα υπέρπυκνο διάλυμα πλάσματος με
αιμοπετάλια, θα μπορούσαμε ενέσιμα να διοχετεύσουμε κατά αναλογία μια τεράστια
ποσότητα αυξητικών παραγόντων στο σημείο της βλάβης, όπου η θεραπεία καθυστερεί
ή έχει αδρανοποιηθεί. Αυτό το υπέρδιαλυμα θα ανοίξει τον διακόπτη της ίασης
οδηγώντας τα τοπικά αδρανή κύτταρα σε πολλαπλασιασμό και διαφοροποίηση, όπως
και θα προσελκύσει και αλλα κύτταρα στην περιοχή ενεργοποιώντας τον βιολογικό
μηχανισμό που είχε τελματώσει.
Η πρακτική
εφαρμογή της θεραπείας αρχίζει με τη λήψη μικρής ποσότητας αίματος από τον ίδιο
τον ασθενή στον χώρο του ιατρείου, οπως ακριβώς γίνεται μια αιμοληψία για
αιματολογικές εξετάσεις ρουτίνας (εικ. 1).
Το δείγμα του αίματος τοποθετείται
σε μηχάνημα που ονομάζεται φυγόκεντρος (εικ. 2), η οποία το διαχωρίζει μετά απο
διπλή κατεργασία στα συστατικά του2,3.
Απο αυτά συλλεγεται το πλάσμα με τα
αιμοπετάλια, PRP το οποίο έχει περιεκτικότητα αιμοπεταλίων πολλαπλάσια του
αίματος (εικ. 3).
Το PRP αναρροφάται σε σύρριγα (εικ. 4)
και ακολούθως ενίεται στο σημείο της βλάβης, υπό υπερηχογραφική καθοδήγηση για
απόλυτη ακρίβεια, σε τένοντα, σύνδεσμο , μυ ή μέσα σε άρθρωση. Η όλη διαδικασία
γίνεται στον χώρο του ιατρείου, διαρκεί μια ώρα και δεν απαιτεί καμία
προετοιμασία πριν, ή περιορισμό μετά απο αυτήν.
Πολλές
επιστημονικές μελέτες έχουν δείξει την δυνατότητα του PRP να επιταχύνει την
διαδικασία επούλωσης μυοσκελετικών βλαβών. Τα ενθαρρυντικά αυτά αποτελέσματα
οδήγησαν στην εφαρμογή της θεραπείας στις περισσότερες ορθοπαιδικές παθήσεις
όπως: μυικές θλάσεις, τενοντίτιδα, χονδροπάθεια και οστεοαρθρίτιδα. Ειδικά
εμφανίζει εξαιρετικά αποτελέσματα στην τενοντίτιδα-περιαρθριτίδα ώμου, τις
επικονδυλιτιδες του αγκώνα,την τενοντίτιδα καρπού,την τροχαντηρίτιδα του ισχίου,
την ιερολαγονιτίδα, την τενοντίτιδα του γόνατου, την τενοντίτιδα του αχιλλειου,
την άκανθα της πτέρνης, τα διαστρέμματα και τις μυικές θλάσεις4,5,6.
Με καθοδήγηση ακτινοσκοπικόυ μηχανήματος εφαρμόζεται σε παθήσεις της
σπονδυλικής στήλης, ενώ πρόσφατες μελέτες έδειξαν την υπεροχή του PRP σε
σύγκριση με το υαλουρονικό οξύ για την θεραπεία της οστεοαρθρίτιδας γόνατου και
ισχίου7,8.
Λόγω του
βιολογικού χαρακτήρα της, η θεραπεία ακολουθεί τα φυσικά στάδια της ιάσης,
συνέπεια τούτου δεν εμφανίζει την άμεση ύφεση των συμπτωμάτων οπως γίνεται με
τις φαρμακευτικές αγωγές, των οποίων η δράση φθίνει με την διακοπή τους. Σε
αντίθεση η θεραπεία με το PRP μπορεί τις πρώτες ημέρες να προκαλέσει μια ελαφρά
επιδείνωση των συμπτωμάτων για να συνεχίσει με αισθητή βελτίωση της κλινικής
εικόνας ειδικά απο την έβδομη ημέρα, να μεγιστοποιηθεί το αποτέλεσμα μεταξύ της
τέταρτης και πέμπτης εβδομάδας ενώ η δράση της διαρκεί απο έξι εώς εννέα
συνεχούς μήνες.
Επειδή το PRP
παράγεται απο το αίμα του ίδιου του ασθενούς δεν υπάρχει κίνδυνος για αλλεργική
αντίδραση ή μετάδοση νόσου. Η ένχυση μπορεί να επαναληφθεί σε μια εβδομάδα και
ανάλογα με την βαρύτητα του προβλήματος να φθάσει μέχρι και τις πέντε ενχύσεις.
Το πρωτόκολλο θεραπείας όπως και η συχνότητα της, εξατομικεύεται για τον κάθε
ασθενή, για την πλειονότητα αρκεί η μια και μόνο ενχύση για θεραπεία.
Αντενδειξείς
εφαρμογής του PRP αποτελεί η λήψη αντιπηκτικών, ο χαμηλός αριθμός αιμοπεταλίων
στο αίμα, ο αρρύθμιστος σακχαρώδης διαβήτης και η ενεργός νεοπλασία. Βασική
προϋπόθεση ειναι να μην λαμβάνονται αντιφλεγμονώδη φάρμακα δέκα ημέρες πριν και
δέκα ημερες μετά την θεραπεία, όπως και κορτικοστεροειδή τριάντα ημέρες πριν
και δέκα ημερες μετά την θεραπεία, διότι αναστέλλουν την φυσιολογική διαδικασία
της επούλωσης.
Συμπέρασματικα
ο στόχος της θεραπείας με το PRP ειναι να οδηγήσει στην ίαση μέσω της πλήρους
φυσικής αποκατάστασης. Οι επιστημονικές μελέτες οπως και η κλινική εμπειρία
δείχνουν οτι οι ασθενείς απαλλάσσονται οριστικά απο οξείες και χρόνιες
μυοσκελετικές παθήσεις. Απεικονιστικός έλεγχος με υπερηχογράφημα και μαγνητική
τομογραφία δείχνει την οριστική αποκατάσταση της βλάβης επιβεβαιώνοντας την
επούλωση του πάσχοντος ιστού. Η άμεση εφαρμογή της θεραπείας αμέσως μετα τον
τραυματισμό μπορεί να μειώσει την ανάγκη χειρουργικής θεραπείας κατακόρυφα,
όπως και να μειώσει τον χρόνο ανάρρωσης σε σημαντικό βαθμό.
Βασικά σημεία:
- Το PRP παράγεται από το αίμα του ίδιου του πάσχοντα.
- Το PRP αποτελεί μια υπέρ συγκέντρωση αιμοπεταλίων και αυξητικών παραγόντων που ενεργοποιούν και επιταχύνουν την διαδικασία της επούλωσης.
- Μελέτες έδειξαν πολύ καλά αποτελέσματα στην εφαρμογή του PRP για την θεραπεία θλάσεων, τενοντίτιδας, χονδροπάθειας και οστεοαρθρίτιδας.
- Η θεραπεία γίνεται στον χώρο του ιατρείου και διαρκεί μια ώρα χωρίς προηγούμενη προετοιμασία η περιορισμούς μετα απο αυτήν.
- Δεν επιτρέπεται η λήψη αντιφλεγμονωδών και κορτικοστεροειδών κατα την διάρκεια της θεραπείας.

The Power of Platelets
Platelet-rich plasma therapy can remedy tough-to-treat injuries

Tiger Woods caused a stir when he admitted that he used it for his knee. Football player Hines Ward used it just before helping his team win the Super Bowl. Pro basketball player Kobe Bryant and tennis star Rafael Nadal have also tried it. But platelet-rich plasma therapy isn’t just for elite athletes anymore.
Now, if we’re sidelined by a stubborn orthopaedic injury or condition such as tennis elbow, Achilles tendonitis or a sore heel brought on by plantar fasciitis, even those of us among the masses can turn to platelet-rich plasma therapy (PRP), a controversial treatment that has led to dramatic improvement in some, but not all patients.
Part of PRP’s appeal lies in its low-risk approach. Medical experts have long recognized the value of minimally invasive treatments that work with the body’s natural healing process, and PRP seems to fit the bill by, in effect, using injections of patients’ own blood as medicine. For treatment of an orthopaedic injury, a typical PRP procedure requires about a syringeful of the patient’s blood, which is spun in a centrifuge to separate the platelets from other components of the blood. A doctor then injects the platelet-rich layer of blood directly into the patient’s injured area. Most patients require only a one-time injection session, and because the patient’s own blood is used, risk of adverse side effects is minimal, says Joshua A. Siegel, MD, director of sports medicine and orthopaedics and an orthopaedic surgeon at Access Sports Medicine and Orthopaedics in Exeter and other NH locations.
PRP was first developed in the 1970s, according to the American Academy of Orthopaedic Surgeons, and appears to be gaining acceptance among orthopaedic specialists. Today, it has practically become de rigueur among professional athletes, who “almost routinely” use it, says Siegel. “It’s really everywhere now in professional athletics, in every sport you can think of,” he says. Pitchers turn to it for elbow or shoulder problems, for example, and football players commonly use it for knee injuries. Using traditional methods, such injuries might have been treated with medication, physical therapy or even surgery, followed by a long recovery. PRP, in contrast, can bring faster healing and carries less risk than some conventional treatments.

Orthopaedic doctors already use PRP to help heal a variety of injuries, but a new application is being explored: treatment of osteoarthritis, a common form of arthritis that occurs when the cartilage that cushions a joint wears down, causing stiffness, pain and loss of flexibility.
“We inject [PRP] right into the osteoarthritic joint,” says Joshua A. Siegel, MD, director of sports medicine and orthopaedics and an orthopaedic surgeon at Access Sports Medicine and Orthopaedics in Exeter and other NH locations.
“Some of the studies coming out show that PRP works much better than some of the treatments that have been used for years,” Siegel says.
In fact, one recent study showed that not only does PRP possibly help to regenerate cartilage, it appeared to prevent breakdown of additional cartilage in 70 percent of the people who received PRP, which is significant because ordinarily those patients would be expected to lose more cartilage over time.
The results raise the possibility that one day PRP will be used as a preventive measure in arthritic patients who exhibit only early, mild symptoms, Siegel says. Stay tuned.

By delivering a highly concentrated punch of platelets directly to an injury, PRP takes advantage of the body’s own regenerative powers to repair damaged bone and soft tissue. Indeed, exploiting the growth factors that occur naturally in platelets and stimulate the healing process fuels the healing potential of PRP. “The growth factors send signals to the body, [in effect saying,] ‘Hey, come over here,’” and prompting the body to jump-start the healing process, says Siegel.
Studies of PRP offer a mixed bag, however, with some research confirming PRP’s effectiveness while other research shows lackluster PRP results. And because PRP is still considered experimental, it is not usually covered by insurance. Patients footing the bill for PRP can expect to pay anywhere from $300 to $800, depending on the practitioner they choose to see.
But some of the patients who have been willing to take a gamble on PRP have achieved impressive results, says Christopher E. Gentchos, MD, an orthopaedic surgeon specializing in foot and ankle surgery at Concord Orthopaedics in Concord and Derry, and at Concord Hospital.
Gentchos remembers the case of a female patient who suffered from plantar fasciitis for two-plus years. After trying multiple non-surgical measures, the patient had foot surgery with a reputable podiatrist. “This [podiatrist] knew what he was doing. The surgery just didn’t work,” says Gentchos. The only thing the patient hadn’t tried was PRP. “I explained that [PRP is] considered experimental, so there were no guarantees and it would be out of pocket. She did it and I would say achieved close to 90-95 percent resolution of her symptoms.” Such dramatic results do not always occur with PRP, Gentchos cautions. “I’ve known folks who had a lower extremity injury, tried PRP, and ended up having surgery anyway. But for some, with PRP they get significant improvement.”
Because our bodies regularly produce new platelets, PRP is suitable for patients of just about any age. “This is one of the few treatments where age doesn’t seem to be terribly important,” Siegel says. “Platelets [in the body] are continually turning over, so it’s not like a 20-year-old’s platelets are better than a 60-year-old’s platelets,” Siegel says.
And PRP can be used to treat a range of orthopaedic woes, although it tends to work best when applied to conditions that are located in parts of the body that often don’t readily heal. Lingering pain from an injury and sluggish healing are very common in certain areas, such as the shoulder and elbow, Gentchos says.
In addition to the out-of-pocket cost and apparently uneven track record, another downside to PRP is the discomfort that comes with it. The sting at the injection site is like that of getting a tetanus shot, Siegel says. Some doctors numb the area, but others do not, fearing that the numbing medicine might somehow interfere with the platelets’ healing effect. Researchers have yet to determine whether those fears are grounded or not. Regardless, the soreness can last for weeks, and in many instances following PRP treatment, patients are instructed to wear a sling, brace, or walking boot for a few weeks depending on the location of the treatment area, Gentchos says.

Platelet-rich plasma guided injections: clinical application in peripheral neuropathies

Platelet-rich plasma (PRP) is defined as an autologous concentrated preparation of platelets and their associated growth factors in a small volume of plasma. The presence of these growth factors has stimulated the scientific community to search about possible benefits of the use of PRP in tissue regeneration. Provided that previously in vitro and animal research demonstrated that PRP could probably play an important role in the treatment of neural tissue disorders, we aimed to review the current literature, regarding the clinical studies that have been conducted to confirm this hypothesis. More specifically, we have reviewed the literature concerning the clinical application of PRP in peripheral neuropathies and investigated if there is strong evidence to establish the use of PRP in clinical practice as a therapeutic option. In contrast with animal studies, we have been able to identify only few clinical data concerning the use of PRP in peripheral neuropathies. We found five trials matched to our research that yields positive and promising results for the future for the application of PRP for the therapy of disorders of the peripheral nervous system. It is obvious that this interesting field of research gives to the scientists the ability to expand it extensively, in terms of both quality and quantity.

Platelet-Rich Plasma Gaining Traction With Some Pain Physicians
Is platelet-rich plasma (PRP) a viable treatment option in patients who are not showing improvement with long-term corticosteroid use? Richard Rosenthal, MD, believes so.
The founder and medical director of Nexus Pain Care in Provo, Utah, Dr. Rosenthal told attendees at the American Society of Interventional Pain Physicians’ 2014 annual meeting that studies support PRP’s efficacy for several pain conditions.
